HITH Care Coordinator- HITH level 2
Based in Gippsland, Latrobe Regional Health are looking for a HITH Care Coordinator to join our Hospital in the Home/Post Acute Care team on a part time, ongoing basis.
HITH incorporates Post-Acute Care nursing programs and other clinics, liaising with internal and external stakeholders. Our Hospital in the Home/Post Acute Care team provides specialised nursing care for people delivered in the comfort of their own homes.
The HITH Care Coordinator shall provide leadership and management to HITH team in partnership with NUM to deliver hospital and community care to enhance patient’s wellness.

We are looking for candidates with:
- Registered Nurse with current Aphra registration
- Three years post graduate experience in Acute setting
- Excellent interpersonal skills and the ability to work well as part of a team
