Remuneration:
Exercise Physiologist Level 3: $2,081.21 - $2,148.65 per week
Dietitian Level 3: $2,081.21 - $2,148.66 per week
Clinical Nurse Specialist Grade 2: $2,107.09 – $2,176.29 per week
Psychologist: $1,432.42 - $2,096.58 per week
Physiotherapist Level 3: $2,081.21 - $2,148.66 per week
Occupational Therapist Level 3: $2,081.21 - $2,148.66 per week

About The Opportunity
ACCP are currently looking for an amazing Care Coordinator to join our team/family.
The ACCP team is a small but mighty multidisciplinary team made up of Aboriginal Health Workers/Practioners, Care coordinators, Dietitians, Admin, CNC’s, Clinical Team Leader, Facility Manager and ACCP Program Manager.
The team covers the whole SWSLHD with 3 office/clinic sites across the district – Wellama (Bankstown), Budyari (Miller) and ACCP offices (Miller).
The team has a big focus on not only the wellbeing of our clients but also each other.
The team works collaboratively in the provision of specialist services to Aboriginal patients and their families with chronic and/or complex needs in a culturally safe, patient centered way to facilitate optimal health outcomes.
The position aims to ‘Close the Gap’ in health disparities for Aboriginal people by addressing their chronic and complex health conditions and contributing factors.
What You'll be Doing
The position is responsible for the provision of culturally safe, patient centred-care coordination services to Aboriginal patients and their families with chronic and/or complex needs. The position will work collaboratively with an Aboriginal Health Worker to provide assessment, care coordination, and education to patients, families and carers that take into account clinical, psychosocial and cultural needs.

How to Apply

If so, please tell us about your Aboriginal and/or Torres Strait Islander heritage, including a brief description of your family and community connections.
  • Current registration with AHPRA as a practicing Registered Nurse, Psychologist, Physiotherapist or Occupational Therapist or professional qualifications in Social Work, Exercise Physiology or Dietetics which provides eligibility to the relevant professional association.
CNS Grade 2: 3 years relevant Full Time equivalent post registration experience and approved post graduate nursing/midwifery qualifications in a field relevant to the specialty field or such other qualification or experience deemed appropriate by the organisation. * Demonstrated understanding of the range of factors that have influenced the health status of Aboriginal communities
  • Demonstrated extensive post graduate experience in the delivery of culturally safe chronic and complex health care coordination services for Aboriginal communities.
  • Demonstrated ability to effectively communicate and engage with patients/consumers, carers and the multidisciplinary health Care Team to identify and ensure responsiveness of care needs
  • Demonstrated capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise
  • Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based chronic disease management services
  • Demonstrated understanding and participation in risk management, work health and safety and quality improvement in the workplace
  • Demonstrated experience with effective health coaching techniques and realistic goal setting for patients with complex needs.
  • Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
