Westmont Aged Care Services is a community based not for profit organisation providing services for both community care and residential aged care.
This is an exciting opportunity for an experienced and dedicated Care Manager to join our team at Westmont Community Care based in our High Street office in Wodonga. The position can either be 5 days per week or 4 days per week based on the successful candidates preference.
You will be responsible for the holistic case management of Westmont Home Care Package client needs, goals and preferences.
You will work collaboratively with our team, clients and their families/carers to ensure clients are at the centre of every discussion, ensuring individually tailored care plans, referrals and services are coordinated, records are current and budgets are carefully managed.
Qualifications & Experience
Essential
- Demonstrated experience in case management and referral
- Tertiary qualifications relevant to aged care, social work, case management or a nursing background
- Aged Care experience
- Commitment to work within consumer directed care and person centred framework
- Sound knowledge of Aged Care Quality Standards
- Friendly, helpful and cooperative team player
- Current Drivers’ License
Demonstrated abilities
- Demonstrated ability to work autonomously with a high degree of initiative
- Demonstrated ability to work effectively with diverse people
- High level computer literacy and client management systems
- Excellent communication skills, both written and verbal
SCHCDSI Award 2010 – Level 3 to 5 depending on experience, with salary packaging of up to $15,900 per year towards living expenses, up to $2600 per year for meals and entertainment as well as novated leasing on vehicles.
