• Case manager for Home Care Package Consumers
• Full time position
• Office location - Sydney

• Your main duties will include:

• Delivery of case management that is person centred and focused on enablement.
• Utilising evidence based practice to deliver quality care and service
• Develop positive working relationships with clients and within the organisation to facilitate improved outcomes for clients.

About you

• Experience with case management for Home Care Package consumers
• Minimum qualification - Diploma in community services
• Knowledge of various funding sources (i.e. Commonwealth Home Support Program, Respite Care and Community Nursing)
• Positive attitude and can do approach for tasks
