We find ways to make people's lives better.About the role – Max Term Contract Part-time 0.6FTE until 31st December 2024, Tuesday - Thursday (8.30am - 4.36pm)In preparation for referrals for our program commencing in January / February 2024, we are looking for a Case Manager to work with young people disengaging from education. Based at our Northern site in Salisbury, you will work together with families, schools, community groups and businesses to create a holistic support approach to achieve our client’s goals. Your duties include but not limited to:Working with young people to create positive outcomes in accordance with their goals and program objectivesManaging a case load of up to 25 clients at a timeDeveloping and managing case plans for students accessing our servicesLiaising with differing service providers such as, education or training facilities, and allied healthComprehensive note taking to ensure client files are up to dateAbout you:You truly believe in the ability to foster and nurture positive relationships with young people. You are enthusiastic, approachable and have a sound understanding of resources available to young people to support their education, health and personal needs. Having a larger case load of students, you will understand the importance of effectively managing competing priorities, but still dedicate time and detail to ensuring case notes are documented appropriately after all client interactions.
