An exciting opportunity is available for a reliable and motivated Client Coordinator to join our Brightwater At Home team. This is a full time position, working Monday to Friday working in the community travelling throughout the Perth metro area.
You will be working predominately from home with assistance provided to set up a home office - with access to Brightwater Aged Care facilities for administrative requirements.
You will be working as part of a multidisciplinary team and responsible for the promotion, assessment, planning and coordination of services that actively deliver choice, independence and quality of life for clients in their own homes.

To be suitable for this role, you must possess:
- Significant experience in a health, aged or community service environment with an understanding of home care packages, CHSP guidelines and Aged Care Quality Standards
- Excellent written, verbal and interpersonal communication skills with the ability to inclusively communicate with a diverse range of people, building and developing purposeful relationships and collaborative teamwork.
- Excellent customer service focus and strong person-centred skills with the ability to effective engage clients requiring services.
- Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines.
- Current Western Australian ‘C’ Class driver’s license
- National Police Certificate (obtained within the last 12 months).
- Working knowledge of occupational safety and health legislation; Aged Care Act (1997); Aged Care Quality Standards (desirable).
- Experience using client management software systems and experience with case management (desirable).
