About the role

Are you an energetic person with a passion to coordinate high quality care to our elderly clients in their homes?

We are looking for an experienced and compassionate Client Services Coordinator to join our team, in a full-time capacity. This role is designed to coordinate the delivery of approved home care packages and private clients for the Ocean Gardens Village. 

As a Client Services Coordinator, you will:

  • Develop, maintain, and review care plans, budgets and statements ensuring alignment with delivery of services for Home Care Package and Private clients.
  • Work closely with the rostering and nursing team to ensure the services delivered are reflective of care plans and budgets.
  • Liaise with medical practitioners and allied health professionals to improve and maintain health and independence for clients. 
  • Liaise with external contractors as part of the Home Care Package coordination. 
  • Maintain client onboarding and leave through Department of Health - Aged Care portals.
  • Undertake other duties as required/directed as service needs change or grow (consistent with the above, within the scope of position and any applicable duties list and any other legal or industrial obligations).

About you

To join us, you will need:

  • Current Registered Nurse License and registration Certificate for Australia (AHPRA)
  • Home Care Package coordination experience and demonstrated skills of clinical experience.
  • Bachelor of Nursing, or equivalent qualification
  • All Ocean Gardens staff must satisfy the relevant probity checks required by legislation and Ocean Gardens policies.
  • Possession of a current driver’s license
  • Demonstrated knowledge and interest for aged persons in their home environment.
  • Sound Organisational and time management skills.
  • Excellent communication and interpersonal skills.
  • Able to demonstrate an innovative and creative approach to service delivery.
  • Knowledge of Work Health and Safety in a home care environment.
  • Able to demonstrate competent computer skills (Word, Excel, Email).
  • Ability to work without supervision, and to use initiative to resolve problems.
  • Ability to work effectively under pressure.
