Our Home and Community team is key in providing care to a range of clients, including older people and those with disabilities. Our teams provide support and services that enable our clients to maintain their independence - living happily and healthily within their own homes.
We are looking for a confident and passionate Client Liaison/Team Leader to join our team! This is a dual role working half of the time as a Team Leader supporting our Home Care Workers & Cleaners and 50% of the time as a Client Liaison based at our Hervey Bay service.
A typical day in your new role:
- Provide supervision, leadership & guidance to Home Care and Support Service workers
- Assist with the orientation of new staff
- Undertake assessments, support planning and package care coordination
- Undertake case management activities
- Supporting the clients with their budgets
- Certificate IV in Home and Community Care or equivalent
- Current National Police Certificate - or willingness to obtain
- Current Drivers Licence and own reliable vehicle
- Proven understanding of assessments, packaged care and case management
- Ability to engage clients and work effectively with them to achieve identified goals
- Support and guidance to clients with budget management and financial sustainability
- Experience in monitoring and assessing services provided to ensure they are following service models and guidelines
- Exceptional communication skills – both written and verbal – and the ability to relate to a wide range of people
- Ability to advocate strongly for your clients and the services they require
- A sound understanding of the community care system and the people who utilise this service
- Diplomacy and negotiation skills; people management capabilities
