Salary per hour: $51.76 + superannuation + generous salary packaging options
Employment Type: Ongoing part-time 48 hours per fortnight

Role description
With a focus on older adults, this is a dynamic role where every day is a chance to make a positive impact and be a health champion. The Community Health Nurse – Living Well role will include the development and implementation of health education programs for individuals and groups, collaboration with local leaders and organizations to promote preventive healthcare measures, participation in community outreach and health promotion events. Liaising with internal stakeholders, conducting health assessments and screenings in the community, providing support for patients with chronic conditions, with the ultimate aim of empowering individuals to take charge of their health.
This position is classed as a ZJ1 as per the Proposed Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2020-2024
About you
As the Community Health Nurse – Living Well, you will be passionate about promoting wellness and preventing illness in your community. You will hold a Bachelor's degree in nursing, have proven experience in Community Health nursing, sound knowledge of public health principles and community resources and strong communication and interpersonal skills.
