Quality Lifestyle Support has been supporting people since 1998 in the Darling Downs Region offering in home support to people who live with the impact of disability.
As a Coordinator you would be responsible for coordinating your team of staff to provide exceptional care to clients in your caseload.
This would include the following:
Client Relationship Management
- Planning the structure of the care for Clients
- Reviewing risk assessments, safety plans and care plans
- Reviewing and maintaining care plans
- Planning activities for Carers to do with Clients.
- Maintaining Relationships with both clients and client representatives
- Interacting and liaising between Clients and Staff in relation to care
- Reviewing shift notes/case notes
- Producing Care Reports
General Administration tasks
- Maintain internal filing and record administration.
- Ensure all client files and records are error free and updated regularly.
- Attend internal meetings.

What we are looking for:

- Advanced computer skills (Microsoft, Rostering, Email, Internet)
- Excellent communication skills with strong people focus.
- Ability to work to deadlines.
- Formal qualification in Disability industry or similar preferable
- High-level accuracy
- Highly self-motivated and able to work autonomously.
- Ability to be adaptable and flexible to meet changing priorities and demands.
- Great communicator that is comfortable speaking on the phone and in person with clients, families & carers.
- You have a flair for remembering people and building relationships.

- Full Australian driver’s license.

- First Aid and CPR ( or willing to obtain)
