What you'll be doing
The Transitional Aged Care Program (TACP) Coordinator has coordination and leadership responsibility for the TACP team. The role encompasses day to day coordination of staff and case management for the consumer admitted to TACP, ensuring compliance with Commonwealth, NSW and LHD Guidelines.
Selection Criteria
- Relevant qualifications and/or knowledge/experience working in a Health/Community Service field and demonstrated skills in Case Management/ Care Coordination with consumers in a community setting.
- Demonstrated experience in Comprehensive Assessment, Care Planning and Case Management with older consumers. An understanding of the process of ageing and the continuum of care for the older consumer including client focused goal setting.
- Demonstrated experience in liaison with General Practice and Primary Care to achieve effective communication, consumer outcomes and transfer of care.
- Demonstrated leadership and coordinating skills within a multidisciplinary team. This would include liaison with NGO service providers and their staff who are considered part of the broader TACP team.
- Demonstrated high level of communication, negotiation, computer and data reporting & analysis skills.
- Proven ability in prioritising workloads to meet competing demands.
- Current unrestricted drivers’ licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances.
