You will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home based settings, making a true difference to our client's independence and well-being.About the teamThe team is an established and progressive multi-disciplinary team which includes Dietetics, Occupational Therapy, Physiotherapy, Podiatry, Social Work and Speech Pathology services, supported by Allied Health Assistants.A great opportunity to experience the public and community sector with a supportive team who can equip you with the skills needed for a strong career start. In this role you will work autonomously, undertake a varied and diverse workload and provide interventions in predominately home based settings, making a true difference to our client's independence and well-being. There is no nursing home work.About the roleThe Dietitian will provide person centred care with an opportunity to:
- Travel across the Goulburn Valley region working in home based settings
- Work with clients, carers and families to provide practical solutions to promote good nutrition and independence at home
- Develop goals and care plans to assist clients to remain at home and active in their local community
- Advise on dietary needs and support skill development in areas such as food budgeting, shopping, food choices and meal preparation
- Support clients who require home enteral nutrition (HEN)
- Work with a diverse range of clients and health conditions including chronic diseases and neurological conditions
- Undertake teaching and training opportunities with local and regional service providers
- Bachelor of Nutrition and Dietetics or Masters of Nutrition and Dietetics or equivalent
- Member of Dietetics Australia and participating in the Accredited Practising Dietitian (APD) program.
- Broad knowledge and experience in the delivery of Dietetic services within home, group and community settings to clients with complex presentations, inclusive of complex health conditions, memory impairment, cultural and diverse backgrounds.
- Self motivation with a high level of communication, organisational and time management skills
- Knowledge of the service system, and ability to liaise and advocate across the full spectrum of health and community services.
- Sound interpersonal, written and communication skills, including computer literacy and the ability to produce well-written reports
- Proven organisational and time management skills with an ability to prioritise and manage a diverse caseload
