We are a non-for-profit disability support organisation working to promote social inclusion, and health and wellbeing by empowering people with disabilities to achieve their full potential. We offer uniquely tailored care programs to participants of the NDIS, offering a range of person-centred supports to meet individual needs.
We are a medium sized support organisation, with approx. 60 participant and 70 workers. Our tight knit head office team is based in Wanneroo, so we traditionally only serviced North of the River participants. However, after much growth and demand for our service, we now offer services to participants South and East of the River. This has created a new position in the organisation and an exciting opportunity for the right candidate.
About the role:
Reporting to the Service Manager, the role will be responsible for the day-to-day coordination of a set of assigned participants, majority being South of the River. Tasks including but not limited to:
- Developing support plans in partnership with participants.
- Coordinating all participant care.
- Liaising with external stakeholders, such as allied health professionals.
- Reviewing incident reports and reporting reportable incidents to the NDIS Quality and Safeguard Commission.
- Reporting approved restrictive practices to the NDIS monthly.
- Guiding and mentoring support workers.
- Preparing Services Agreements.
- Matching new recruits to participants.
- Assisting new recruits with inductions.
- Assisting with out of office On-Call duties (loading payable).
Requirements for the role:
- Cert IV in Disability minimum, or other relevant qualification for the role.
- Demonstrated experience as a Coordinator in a similar position
- Proven understanding of the NDIS framework.
- Understanding and experience implementing a consumer-directed care mode.
- Demonstrated ability to lead and support a team.
- Excellent administration skills.
- High attention to detail and ability to multitask and problem-solve.
- Proven ability to work collaboratively with stakeholders, clients and team members.
- Time managing skills.
- Managing competing priorities and deadlines.

What We Require:
- A current NDIS Worker Screening Check or willingness to obtain.
- Working with Children Check, or willingness to obtain.
- Current First Aid and CPR training, or willingness to obtain.
- Reliable vehicle and a drivers licence.
- A positive, reliable, empathetic and proactive approach to supporting people of all ages and backgrounds living with a disability.
