We are currently seeking a dedicated and compassionate Disability Services Officer to join our team, providing high-quality support and services to individuals with disabilities. Key Responsibilities:

• Develop, implement, and monitor individualized plans to meet the needs and goals of service users.
• Provide direct support to individuals with disabilities, including personal care, social support, and assistance with daily living activities.
• Facilitate and promote the participation of service users in community activities and events.
• Work collaboratively with service users, families, carers, and other professionals to ensure a coordinated and integrated approach to service delivery.
• Maintain accurate records and documentation in accordance with organizational and legislative requirements.
• Participate in continuous improvement activities and training to enhance service delivery.

Essential Criteria:

• Diploma qualification in Disability Services, Community Services, Mental Health, or a related field.
• Proven experience in providing support to individuals with disabilities, including developing and implementing individualized plans.
• Excellent communication and interpersonal skills, with the ability to build rapport with service users, families, and colleagues.
• Strong organizational and time management skills, with the ability to prioritize tasks and work independently.
• A commitment to the principles of social inclusion and the empowerment of individuals with disabilities.
• Current First Aid Certificate and Working with Children Check.
• NDIS Workers Clearance
• Valid driver's license and reliable transport.
