About Your Role:
We are looking for experienced Support Workers with a focus on Prader-Willi Syndrome (PWS) and High Needs support. The responsibilities include the following:
- Supporting individuals to gain independence in everyday life and learn new skills
- Connecting individuals with recreational, sporting or community groups
- Collaborating with key people in an individual's life, including family members, employers or other support services to achieve the best outcomes
- Role model and advocate for people living with disability in the community
- Build rapport and collaborate effectively with participants and their support network, including staff/management, families and friends
- Implement Individual Plans and Positive Behaviour Support Plans
- Data Collection of support needs and behaviours of concern
- Engage participants appropriately in activities of preference
- Provide Personal Care support
- Support participants to engage in community access to build skills and capacity
- Work with challenging and complex participants with behaviours of concern
- Transport participants
Required Attributes:
- Sound PWS experience
- Experience supporting individuals with Mental health and complex challenging behaviours
- Experience in personal care support
- Creativity and outside-the-box thinking
- Ability to adapt your mentoring skills to people from diverse backgrounds
- Ability to create and maintain professional working relationships
- Ability to work autonomously and within a team
- Excellent communication and interpersonal skills
- Be flexible and adaptable to individual needs
- Be organised and have good time management skills
- Flexibility with hours is essential as is availability to work weekends
Applicants must have:
- Working with Children Check (WWCC)
- NDIS Worker Screening Check
- First Aid Certificate
- Drivers Licence
- Reliable Vehicle and car insurance
