This Exciting Role
The For Care Service Coordinator is responsible for overseeing the daily operations of Supported Independent Living program, in home and community access. The ideal candidate will have significant experience with NDIS and management expertise. You will need to have a strong understanding of team goals whilst ensuring these goals are serving the strategic vision of the organisation. You will create and maintain transparency within the program which allows all relevant stakeholder to respond and contribute appropriately.
Responsibilities
- Implement and maintain effective communication and information management systems
- Exercise initiative in the application of established work procedures for the benefit of Service Users
- Meet and greet with Participants/Clients/Family to discuss individual needs, likes, interests, support worker preferences
- Offer consultation/guidance from the Disability Support Workers and Key Workers in relation to Service User needs assessed as outlying defined established practices
- Manage budget as per Purchasing Guidelines
- Maintain and build relationships with existing clients and key referrers such as Support Coordinators, Plan Managers, and Allied Health Professions
- Instigate emergency intervention for emergency situations (call 000)
- Apply first aid interventions (per the scope of Senior First Aid certification)
About You
- Degree qualified, preferred; or Certificate III in Disability with considerable experience
- Proven experience in Disability Service Leadership (3 – 5 years desirable)
- Current First Aid Certificate
- Current Police Clearance
- Working with Children Check (Blue Card) and NDIS Workers Screening
- Awareness of NDIS polices and procedures
