We are currently recruiting an experienced Care Manager to join our friendly and dynamic team in our Braeside office. Part-time or full-time is negotiable. Potential career advancement is available if you are ambitious and want to grow into the future with the company.
ABOUT THE ROLE:
Your responsibilities may include but are not limited to:
- Providing case management support in accordance with the Aged Care Act 1997 and CSAH Policy & procedures, within set timeframes, according to set targets and individual consumer budgets.
- Liaising with internal and/or external services/agencies to improve the implementation of care plans for clients and to provide services where required.
- Developing and reviewing comprehensive person-centered care plans in consultation with consumers, their nominated representatives and relevant stakeholders within individual consumer budget parameters.
- Brokering required services to ensure timely provision of relevant supports and services through written and verbal communication with all parties and within budget.
- Maintaining effective links with relevant professional networks (e.g ACAS, GPs clinic and community services) Customer Services
- Regularly provide support and advice to consumers and their representatives to enable them to make informed decisions, contribute to their care plan and evaluate the effectiveness of current care plans.
- Advocating on behalf of consumers where required to ensure required services are delivered
- Investigating and responding in a timely manner to consumer inquiries, concerns or complaints and ensuring that consumers are advised of the agreed level of services and any changes to plans.
- Promote CSAH Home Care Packages and provide information to prospective consumers.
- Promote and enforce CSAH’s policy of excellence in customer service & service Provision Support
