About Us
Emerging Futures Collaborative Limited (EFCL) is an initiative established by a collaborative of Catholic Religious Congregations in Australia to address an emerging need in Australian religious life for transition and support services. EFCL is led by a small core team of experienced staff who are designing, building and operating this innovative service as it grows.
The Missionary Franciscan Sisters has partnered with EFCL to provide support services, and we are seeking to appoint a Health and Wellbeing Coordinator to coordinate Health and Wellbeing care and support services to members of this religious institute who are based in Brisbane.
The Opportunity
Reporting to EFCL’s Care Coordinator, and working in close collaboration with the Missionary Franciscan Sisters, the Health and Wellbeing Coordinator supports sisters living independently or in community and in residential aged care with regard to their Health and Wellbeing needs.
You will ensure a comprehensive suite of care and support services are provided to the sisters, enabling them to live their life to the full, and enabling them to continue their participation in the congregation’s mission and community.
This position is a permanent part-time role working 22.8 hours (3 days) per week, and hours can be worked flexibly across the week. The role is based at the Congregation Office at Kedron, Brisbane. Attendance at sisters’ homes and where they reside in residential care, will be required. Some travel to Sisters who live further away may be required occasionally.
About You
Your experience in a case management or care coordination role, as well as your appreciation for the mission, spirituality and ethos of the Missionary Franciscan Sisters will be key to this role.
The Ideal candidate will also demonstrate the following Key Requirements:
- Relevant health or allied health qualification with experience in an aged care, hospital or community care setting
- Experience in case management or care coordination
- Good understanding of the aged care sector and familiarity with government funded services and how to identify and make best use of them
- Capacity to support individuals through various transitions which might include
navigating community services and systems such as sourcing care packages
- Flexible approach to working required hours and ability to respond to emergency calls
- Empathy, diplomacy and high-level interpersonal skills that support respectful relationships and the ability to advocate for individual’s care needs
- A current Drivers Licence and use of a car
- Evidence of Covid-19 and Influenza vaccination immunisation
