Western Queensland PHN (WQPHN) is one of seven Primary Health Networks across Queensland, and 31 nationally funded by the Australian Government, which has a supportive, innovative and collaborative organisational culture. Our goal is to improve the integration of primary care services to create seamless health care in our shared communities across almost one million square kilometres of Western Queensland.
WQPHN is a commissioner of health services who works closely with all health service providers across the region to identify where there are gaps in primary health care, and commissions local providers to cost-effectively provide those services, building a sustainable and integrated system that places the patient at the centre of care.
The RoleTo support the Healthy Outback Communities (HOC) initiative, the purpose of this role is to develop and implement Health and Wellbeing solutions that align with outback communities' goals. This role involves collaboration with the HOC Alliance partners and other service providers to co-create and negotiate service and activity responses that are culturally appropriate and context-specific. The Health and Wellbeing Coordinator fosters strong relationships and effective communication channels between all stakeholders, ensuring that service solutions are innovative, practical, and sustainable. The role also involves supporting the HOC Wellbeing Connector in coordinating these services at the community level.
The role is responsible for the following key functions within WQPHN:
- Conduct effective planning, design, and implementation of Health and Wellbeing services and initiatives in line with personal and community wellbeing plans.
- Foster collaborative relationships with local communities, health service providers, and other stakeholders.
- Contribute to the continuous improvement of the HOC program through feedback and innovative ideas.
- Uphold the principles of cultural sensitivity and inclusivity, particularly when working with Aboriginal and Torres Strait Islander communities.
- Maintain up-to-date records and reports on service delivery and outcomes.
- Provide regular updates to the Senior Manager, Health Improvement & Innovation on key achievements, challenges or potential risks and issues, understanding critical or politically sensitive issues that may require immediate attention and risk mitigation.
- Contribute to employee training and support professional collaborations or forums and quality improvement networks.
- Undertake other such duties and training as may reasonably be required and which are consistent with the level of responsibility of this position.
The successful candidate will possess:
- Tertiary qualifications in health science, public health, health promotion, health management or similar, or equivalent experience in a similar position.
- Experience in designing and implementing Health and Wellbeing service solutions, preferably in rural and remote settings.
- Strong project coordination, health service planning, and/or management background.
- Demonstrated ability to prepare clear and concise business communication documents including reports, budgets, meeting minutes, briefing notes, letters and electronic correspondence.
- Ability to work effectively with minimal supervision, manage conflicting deadlines and demonstrate effective problem-solving skills as part of a regionally dispersed team.
- Highly developed interpersonal communication skills to build and maintain internal and external relationships, with demonstrated excellence in stakeholder engagement, negotiation, and relationship management.
- Competency in Microsoft Office (including SharePoint) and other office productivity tools, with aptitude to learn new software and systems.
Ideally, this critical contract position will be based within the WQPHN region in Longreach, Mount Isa, Winton or Roma; however, applicants are welcome to apply from across the state. Regular travel will be required across the WQPHN patch to successfully perform this role, and hybrid work-from-home arrangements also apply.
