The role
This is an opportunity to learn about Injury Management and returning injured people to health. Through our injury support academy, we will teach you the skills and information you need to be successful in this complex area.
You will then assist injured people in Victoria to return to health by coordinating medical and treatment services over the phone. You will be required to liaise with key stakeholders to help the injured people overcome obstacles and barriers impeding their return to health.
You will empower your patients with the knowledge and tools needed to return to health holistically, help navigate the complexities of the healthcare system, and promote both short and long-term positive outcomes.
In this role, you will:
Provide telephonic support and care coordination.
Take responsibility for telephonic care management intervention and coordination of healthcare services.
Engage providers of health services to facilitate referrals and ensure the timely provision of services.
Liaise with insurers, treatment providers and medical teams to ensure a timely and sustainable return to health of the injured person.
Close potential gaps in care by discussing medications, reviewing discharge instructions, coordinating follow-up appointments and services, addressing patients’ future health needs, and taking other appropriate action to improve return to health outcomes.
Work directly with the Honeysuckle Health team to continuously improve the quality of the services delivered.
About You - Selection Criteria
We are looking for clinicians on a Permanent Full-Time or Part-Time basis (0.8FTE) and your background will include:
A current unrestricted Registered Nurse registration.
A minimum of 3 years post-graduate nursing experience in an acute clinical setting.
Preferred background in telehealth.
A friendly and confident telephone manner, with experience providing clinical care and telephonic advice.
An understanding of or interest in the systems relating to injury management.
A broad understanding of a variety of health concerns, health processes, resources, procedures, and surgeries.
Experience and confidence with the use of digital platforms, e-records and emerging technologies.
Passion for improving health outcomes at a population level, while remaining client-centred.
Strong initiative and ability to work independently and to adapt to change.
Excellent time management & prioritisation skills.
Excellent communication skills – written and verbal.
Culturally sensitive to a diverse patient base.
Currently residing in the state of Victoria
