At PCCS our approach is practical, and strengths based, and we are seeking a like-minded experienced NDIS Support Coordinator to join our exceptional team.

Our mission is to optimise Health and Wellbeing for local communities and support young people and adults with simple to complex support needs including physical, social and mental health issues covering illness and injury.
As an experienced NDIS Support Coordinator you will work in a team to assist clients to work towards their goals in their NDIS plan. You will provide support coordination to clients and assist them with their NDIS applications/Plans. You will also be working operationally in NDIS activities, coordinate with staff and collaborate across reporting lines to ensure successful implementation of NDIS activities within PCCS.
WHAT YOU’LL DO
- Support clients with transition to the NDIS ensuring all necessary NDIS information is fully available to clients in transition and assist in answering related questions.
- Support, represent and advocate for PCCS clients within the community with regard to NDIS matters and support services to optimise NDIS funding.
- Effective collaboration and communication with key internal and external stakeholders regarding NDIS activities including establishing and maintaining effective relationships with existing and new stakeholders.
- Assisting the service delivery team in providing information and support to Services in all NDIS matters.
WHAT TO BRING
- Qualification in Social Work, Social Science, community services, community development, or other relevant areas.
- Understanding of NDIS service and ability to develop a strong understanding of its links to clients and services.
- Valid Drivers License
- NDIS Worker Screening and Blue Card
- Relevant Vaccination Status
