Finding Yellow was founded in 1978 by parents in the Hunter Region, aimed to improve support for individuals with disabilities. Driven by community support and fundraising, they sought a brighter future for their children. In 1982, the Cessnock Hostel was established, providing essential support and accommodation for local clients/participants with disabilities. Renamed "Finding Yellow" in 2013, the organisation remains committed to enhancing the lives of those with disabilities, striving to bring joy and fulfillment to all abilities. Today, Finding Yellow offers comprehensive disability support in Support Coordination (COS), Supported Independent Living (SIL), Independent Living Options (ILO), Community Drop-in Supports, Socials & Supported Getaways.
Qualifications & experience
- Bachelor of Nursing.
- Current AHPRA Nursing Registration.
- Minimum 3rd year RN experience.
Tasks & responsibilities
- Carry out practices in accordance with legislation including the NDIS Act 2013, Registered Nurse Standards of Practice and take action to rectify any unsafe practices or unprofessional behaviour in the workplace.
- Complete and update participant Comprehensive Health Assessment Plans (CHAP) on an annual basis and as required.
- Produce and update Support Protocols from participant Allied Health Plans including Mealtime Management, Positive Behaviour Supports, Mobility etcetera and ensure they are safely and implemented in a timely manner.
- Conduct Clinical Inductions for front-line staff.
- Sign-off on buddy shifts that have been conducted by Senior Support Workers.
- Oversee the administration and conciliation of medications, maintaining quality control including training, competency assessments, documentation, and resolution of medication errors.
- Carry out clinical practice quality control checks and implement continuous improvements through analysis of clinical logs including Falls Logs, Behaviours Logs and Medication Sign-off, random sampling of shift notes etcetera.
- Implement Incident Corrective Actions.
- Attend stake holder clinical case meetings including Medical Appointments.
