Job description

  • Full time 15-month contact (until June 2025) position based in Liverpool 
  • $108k inclusive of Super + Salary Packaging

About the role 

Join us in leading transformative change! As a senior member of our team, you will spearhead specialised and intensive assistance, facilitating access to vital aged care services and fostering seamless integration between the health sector, aged care, and other community agencies from the ground up. Your role will be pivotal in driving enhanced collaboration and connectivity, ensuring vulnerable and isolated older people receive the comprehensive support they need to thrive within our community! 

Duties 

  • Support team members in managing complex cases and client service delivery, assisting during visits, meetings, and documentation, and collaborating to resolve challenging situations. 
  • Conduct comprehensive assessments with clients to develop, implement, monitor, and review client plans and outcomes, focusing on identifying strengths and areas of risk. 
  • Attend various meetings and maintain appropriate documentation, including review meetings, case conferences, and supervision meetings. 
  • Provide regular supervision, coaching, and on-the-job support to Care Connectors to enhance client outcomes and assist with onboarding of new team members.

Skills and Experience  

  • Relevant tertiary qualifications (e.g. social work, human services, aged care, community services or health) and/or significant relevant experience 
  • Local community connections within the aged care community 
  • A current Class C NSW driver’s license 
  • Demonstrated computer skills including Microsoft Office and ability to enter information correctly into databases and/or portals 
  • A detailed understanding and ability to source a full range of aged care supports and services. 
  • A commitment to delivering a person-centred approach that respects and responds to each person’s individual needs, preferences, values and life experiences 
  • An ability to communicate effectively and support the needs of people with diverse backgrounds and life experience, including Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD), and LGBTQIA+ communities
