Providing service coordination to clients who have home care packages.

Role:
This role provides support coordination to elderly clients with Home Care Packages. You will be working as part of a small team of dedicated coordinators. It is a role that requires a good understanding of the aged care sector and the changes that are currently happen in this space. You will be the first point of contact and will require excellent communication skills. This is an integral part of the HCP team and you will be given support and guidance from colleagues and management.
Selection Criteria:
In order to be selected for this role you will require:
- Third level qualification in Aged Care, Case Management or Social Work
- Extensive experience working with elderly people
- Good understanding of HCP
- Excellent communication skills
- Ability to work autonomously and as part of a team
- Experience working with external stakeholders.
