Primacy Care is committed to providing comprehensive support services to NDIS participants living with a psychosocial disability. We are currently seeking a Service Delivery Officer who can bring their industry experience and qualifications to join our Service Delivery team. You will play a critical role in ensuring the delivery of high-quality support services to the participants, empowering them to build their capacity, progress and achievement of their NDIS goals.
About the Role:
The role is centred on focusing on the Service Delivery provided by Mental Health Key Workers/Support Workers to NDIS participants, the key responsibilities will be divided into two different areas.
Essentially, the role will assume the responsibility of ensuring the delivery of exceptional support services to NDIS participants, in a collaborative approach with our other Service Delivery Team. The role includes supervising an area, which encompasses a team of key workers and participants, ensuring that the quality of their service provision, meets organisational standards and participant needs. Your role will involve monitoring, maintaining, and supporting a culture of quality and safeguarding within our organisation.
Key Responsibilities
- Build and maintain strong participant relationships and support networks.
- Adaptive communication skills to meet the diverse needs of each. participant and Key Worker.
- Collaborate with applicable internal teams.
- Uphold the Complaints and Feedback process
- Conduct monthly 1:1 meetings with field staff
- Collaborate with the Rostering Officer to resolve reallocations, shift coverage and provide guidance on Service Delivery and scheduling.
- Monitor the Client Management System, ensuring completion of all shifts and processing weekly timesheets.
- Review and assess submitted care notes daily.
- Document Management to uphold quality and compliance requirements.
- Acting as a communication support point for participants during crises and connecting them to relevant support services.
What You Bring:
- Efficient multitasking and strong organizational skills.
- Previous Industry experience.
- Proficiency in MS Office Suite and Customer Relationship Management systems.
- Strong verbal and written communication skills.
- Relevant industry qualification/certification (Certificate IV and above) is desirable.
