Job description

About the role

  • Liaise with inpatient clinical teams and hospital Palliative Care services to optimise care transitions and home-based care.
  • Provide undergraduate and postgraduate teaching, research, quality assurance audits and continuing education as integral to the position.
  • To liaise with Palliative Care multidisciplinary colleagues across MNH, to streamline the patient's journey and ensuring care is delivered within their chosen location whenever possible.
