About Sunbury and Cobaw Community Health

Sunbury and Cobaw Community Health is a not-for-profit community health organisation providing a comprehensive range of free and low-cost health, wellbeing and community services in early childhood, youth, families, adults, aged and disability.

Covering an area of over 1700 km2 from Sunbury to beyond the borders of Macedon Ranges Shire our 280 staff and 100 volunteers are based across four sites at Kyneton, Romsey, Sunbury and Woodend.

The organisation has a proud history of delivering community health services to the region for over 40 years.

About the role: 

This position is part of our Primary Care Unit, which delivers a wide variety of services and programs through Commonwealth and State Government block funding, ‘fee for service’ programs including NDIS Participants and Home Care Package clients as well as Primary Health Network funding for chronic and complex clients in the Macedon Ranges and Sunbury region.

Work in the local Sunbury, Cobaw and Macedon Ranges community through a range of flexible funding schemes including:

- Community Health

- School Readiness

- HACC-PYP

- NDIS

Applicants who are seeking a substantial School Readiness Funding caseload are strongly encouraged to apply

Key areas of responsibility of this role include:

  • Provide assessment and management of speech pathology clients (birth to 18), using a range of service delivery models including face to face, group programs and tele-health in environments including home, community and centre based.
  • Make appropriate referrals for clients whose service requirements are more suitably catered for by an alternative service provider, or whose requirements necessitate access to other specialist services.
  • Provide information and support to parents/caregivers of children/adolescents receiving speech pathology services.
  • Experience in the delivery of occupational therapy services to children and/or adolescents
  • Experience in working with families using a strengths-based approach.
  • Knowledge of the early childhood and/or education sector.
  • Experience and/or knowledge in the delivery of community education programs and a clear understanding of confidentiality and privacy requirements.
  • Demonstrated ability to review and develop programs and service delivery models that enhance the operations of the Child and Youth Allied Health Unit and SCCH
  • Ability to operate in a consultative manner, within the organisation, community and with key stakeholders.

The successful candidate will be required to undertake and maintain a National Criminal History Check (NCHC), an NDIS Safety Screening check and hold a current valid Working With Children Check.
