Booroongen Djugun specialises in providing Community Care and Residential Care services to Aboriginal and Torres Strait Islander Elders in the Kempsey area, offering a space to access culturally-informed care. We’re also a Registered Training Organisation, offering vocational education and training to staff and community members looking to broaden their horizons and enter the workforce.
The space itself was designed with the totems of fire, water, earth, and sun in mind, in order to support the strong ties to traditional life that many clients of Booroongen Djugun hold.

The Support Coordinator will assist clients to build their capacity to make decisions and choices about the implementation of all supports in their NDIS plan, including mainstream, informal, community and funded supports. The Support Coordinator connects people with a NDIS Support Plan to access a range of supports which align with their NDIS plan, across one or more providers, with a strong focus on seeing participants build capacity to meet their NDIS short-term and long-term goals.
You will work closely with NDIS participants and their families/guardians to connect with individual services and supports outlined in their NDIS Plan, ensuring they get the most out of their funding and achieve desired outcomes. The Support Coordinator must be able to provide a Culturally Safe service to Aboriginal and Torres Strait Islander clients or recognise the need for engagement of appropriate Cultural Safe personnel to deal with any Cultural Safe services and requests including Protocols and traditions.
Key responsibilities and duties
Support Coordination
- Support clients and their support networks to understand their NDIS plan, goals and objectives
- Deliver coordination of supports to individuals with a NDIS plan:
- Source, coordinate and manage complex and intersecting supports to suit individual needs across multiple providers, ensuring client choice and control
- Work with mainstream service providers including negotiating services and prices to ensure maximum value for money is achieved and service obligations are met
- Support clients to build their capacity, resilience and networks with the aim of greater choice and control and independence in managing their personal choices
- Work with clients to access and navigate the My Place portal to establish service bookings and service agreements
- Arrange any assessments needed to determine the nature and type of funding required
- Decide the budget for each support type and advise any relevant plan manager of the breakdown of funds
- Liaise with plan managers to establish the appropriate claim categories and attribute funds accordingly
- Assist clients to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money for their plan, identify solutions to problems experienced in implementing the plan, and to consider new goals
- Assist clients to communicate with the NDIA when required to support their own plan requirements
- Resolve any crisis situations, ensuring that the client’s best interests and resilience are supported.
- Respond to new support co-ordination enquiries for new participants by contacting new clients as soon as possible after handover with the NDIS planner, ideally within 2 days and meet with the client within 5 days
Core Competencies/Requirements/Experience
- Sound cultural knowledge and understanding of engaging and providing cultural safe services to Aboriginal and Torres Strait Islander people
- The ability to engage with Aboriginal and Torres Strait Islander Advisors withing Booroongen Djugun and other Leading Aboriginal services within the Mid North Coast region
- Qualifications is Disability or Community Sector qualification desirable.
- A minimum of 12 months’ experience in support coordination
- Sound knowledge of NDIS and experience working with people with a disability
- Ability to effectively liaise with a range of internal stakeholders
- Understanding of the following Acts, guidelines, standards and bodies:

- NDIS Quality Safeguard Commission
- Disability Practice Standards
- NDIS Code of Conduct
- Awareness and understanding of issues confronting people with disabilities in a home or community setting
- Ability to articulate and advocate for person-centred support, empowerment of people with disability and community inclusiveness
- Excellent interpersonal and communication skills and the ability to work effectively as a team member
- Demonstrated skills and knowledge in the areas of planning, implementing, reviewing and evaluating effective service plans
- Demonstrated ability to case manage several complex cases concurrently, with an emphasis on client life goals
- Possession of good knowledge of local service providers, both specialist and mainstream, and the ability to search and source services to meet participants goals
- Intermediate level proficiency in the use of software and systems including accuracy in data entry
- Ability to demonstrate a high degree of confidentiality and context specific discretion regarding individual privacy
- Proven ability to make decisions, appropriately escalate to executive manager risks and issues, and work independently in accordance with BDL procedures
- Understanding of the NDIS Price Guide and flexibility within budgets
- Completion of the NDIS Worker Induction Orientation and NDIS Worker Orientation Module/s
- Be able to pass the NDIS Worker screener check
