Duties include:
- Collaborating with NDIS participants, their families and carers,
assisting them to understand, implement, manage and review their NDIS Plans. - Promoting the independence of people living with a disability to achieve their life goals by assisting them to establish connections with service providers.
- Regularly monitoring and reviewing plans, recording and reporting outcomes to the Participant and NDIA within required timeframes.
- Facilitating NDIS participants, their families and carers to build capacity to manage their own NDIS plans and exercise choice and control over the supports they receive.
About You:
- Thrive in a fast-paced environment
- Excellent time management and organisation skills with the ability to work autonomously and as part of a team
- Demonstrated high level of computer literacy, numeracy and attention to detail.
- Ability to collaborate with others, provide and accept advice and assistance from colleagues to meet common goals
- Demonstrated highly developed written and verbal communication, negotiation and analytical skills
- Creative and effective problem solving skills
- Support Coordination or Case Management experience and or Disability sector experience preferred
