About you:

- Based in Swan Hill, this is an ongoing Full-time or Part-time role with flexible working arrangements negotiated with the successful applicant.
About the position:
- A Support Coordinators role is to work with participants, their families and broader networks to proactively identify and access the right services to best support individual needs.
- Working from a strength-based approach, Support Coordinators are integral in assisting participants to:
- Fully understand their NDIS plan and advocate for the right levels of support funding
- Access services, supports and plan for the future
- Work creatively to achieve goals, overcome barriers and support to fully participate in all aspects of the community
- Build capacity, develop new skills for greater independence and enjoyment of all aspects of life
Key responsibilities:
- Work with clients, families and carers to identify and access opportunities for skills development, education and employment
- Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plans
- Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports.
- Prepare and maintain up to date and accurate case management notes and reports using clear and concise language on client progress.
- Liaise with a wide range of stakeholders, community services, government agencies, customers and service providers.
- Attend and contribute to team meetings, discussions and planning. Generate ideas and options, whilst maintaining flexibility and receptiveness to change and alternatives.
Skills and experience:
- Experience in NDIS Support Coordination (preferred, but not essential)
- Experience working within a team in the community services sector
- Strong analytical and problem-solving ability
- Highly developed verbal and written communication skills
- Strong organisational and time-management skills
- Outstanding attention to detail
- Commitment to the values of care and empathy
Qualifications:
- Certificate IV/Diploma Community Services or Disability and/or Bachelor degree in Social work or similar related field (or working towards)
- Relevant experience in Community Services, Disability or relatable Health Field.

- A current NDIS workers screening check (or willing to obtain)
- Working with Children check (or willing to obtain)
