You will identify, connect, and negotiate with providers to deliver quality outcomes and timely service whilst working closely with other external stakeholders involved in an individual's care or provision of services.
To be successful in this role, you MUST have:
2+ years' experience in an NDIS Support Coordination role or case management role and be well versed with NDIS systems/regulations/compliance, disability practices and other relevant service models that intersect with your role.
Qualifications in Social Sciences, Human Services or Allied Health.
Great relationship-building abilities
Exceptional organisation and time management skills
Experience in monitoring and achieving billable hours
Excellent written and verbal communication
Ability to work collaboratively with a wide range of stakeholders
Passion and professional drive in the disability sector, to continue developing your skill set and enhance the support coordination framework
A genuine desire to provide the best quality service to everyone you work with.
High levels of comfort dealing with a range of priorities and adapting to new systems and processes.
