The West Moreton First Nations Holistic Wellbeing Service aims to promote clinically appropriate delivery of care in the community for young people and adults at risk of, or living with, chronic health conditions. This will be achieved through building connectedness to family and community, resilience, health literacy, and chronic condition prevention and self-management with an emphasis on social and emotional wellbeing as part of a holistic approach.

About the role
The Team Leader for the First Nations Holistic Wellbeing Service provides operational management and allied health clinical leadership to the First Nations Holistic Wellbeing Service.
This role will work towards the priorities established under the West Moreton First Nations Health Equity Strategy 2022 – 2025 and work closely with colleagues across the health continuum to optimise resources, share innovation and ensure consistent clinical practices and cultural appropriateness of care provision across the hospital and community sectors.

About you
Our ideal candidate will be someone who can demonstrate the following:
- High level knowledge of contemporary preventative and hospital avoidance service delivery with the ability to integrate evidence-based knowledge into practice.
- Ability to lead a multidisciplinary team including First Nation health workers utilising the principle of contemporary human, financial and material resource management.
- High level oral and written communication and interpersonal skills to facilitate equitable and effective engagement with staff and internal and external stakeholders.
- Ability to provide strategic leadership for service planning and service improvement for the service.
- Ability to provide clinical leadership for the development of professional standards for Prevention and Hospital Avoidance practice in West Moreton Health.
- Ability to interpret and analyse data and manage systems reporting at a high level.
- Ability to lead and maintain culturally appropriate aboriginal and Torres strait islander services.
- High level knowledge of barriers Aboriginal and Torres Strait Islander people face in accessing health care and the impact of health outcomes.
- High level knowledge of relevant Aboriginal and Torres Strait Islander policy frameworks.
Mandatory Requirements:

- Under s25 of the Anti-Discrimination Act 1991 (Qld) it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.
