About the role
In this role, you will provide leadership to the Home Care Package team, strengthen key stakeholder relationships in the community and promote an inclusive customer centred approach to care, while ensuring performance measures and indicators are achieved, and meet compliance by maintaining relevant policies, procedures and standards.
Leading and mentoring a team of 6-8 Home Care Packages Case Managers, you will be translating organisational goals into program area goals and promoting an integrated and client centred approach to care.
About you
As an experienced HCP Case Manager with demonstrated operational/staff management experience, you will be familiar with supporting and mentoring a small team of staff, providing growth opportunities and have a proven track record of working successfully through a period of change and growth. For this role, you will require a tertiary qualification in an Nursing or Allied Health, and have a strong clinical background to draw on. You have an open and positive style of communication with your staff and embrace change, new technology and think outside the square.
