About the Opportunity
Miller Community Health Centre is recruiting for a Aboriginal Chronic Care Program Manager (Health Manager Level 3) to complement and enhance the current Community Health Team.
The position is responsible for leading and managing the SWSLHD Aboriginal Chronic Care Program (ACCP), which provides culturally responsive, patient centered care coordination services to Aboriginal patients and their families with chronic and or complex needs. The ACCP Manager will ensure the cultural, clinical and psychosocial components of the ACCP model operate seamlessly to provide holistic care to Aboriginal patients and their families. The role will ensure there is an effective interface between ACCP, the mainstream primary, secondary and tertiary health sectors and the Aboriginal Community Controlled Health sector to facilitate access to appropriate care for SWSLHD Aboriginal Chronic Care patients. The position will be responsible for ensuring ACCP maintains its footprint across the South Western Sydney region and the oversight of its three dedicated facilities and the multidisciplinary team that operate from these premises.
SWSLHD operates its services on the lands of the Dharawal, Darug and Gundungurra peoples and has a rich history of working alongside local Aboriginal organisations and programs. As part of the SWSLHD Aboriginal Workforce you will have access to a wide range of cultural supports across the service including the districts Aboriginal Health Unit, peer supervision, connection to the Aboriginal management group and a large workforce of Aboriginal colleagues.

How to Apply

- This is an Identified Aboriginal/Torres Strait Islander Position. Describe your connections to community and culture.
- Tertiary qualifications in social science, health or related field, or relevant work experience. Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
- Demonstrated experience in leading and managing an Aboriginal health service or program with a multidisciplinary team. This includes direct line management experience.
- Demonstrated experience in developing service networks and collaborative service models that improve access to health services for Aboriginal communities
