About the Care Finder program
Here at EACH we are looking for a Care Co-ordinator to join our Care Finder program which aims to provide specialist and intensive assistance to help people in the Care Finder target population groups to understand and access aged care services and connect with other relevant supports in the community.
Job Description
- Part-time (0.6FTE) max-term position until June 2025
- Work within the community around the Wingecarribee area

As Care Co-ordinator you will practise assertive outreach and explore and establish different ways to engage with people in the Care Finder target populations and connect them to services within the community.
Duties include:
- Engaging and building rapport with potential clients and other general and specialist support services.
- Using a person-centred approach provide specialist and intensive assistance to help senior Australians who need intensive support, who could otherwise fall through the gaps, to understand and access aged care services and connect with other relevant supports in the community.
- Building and establishing relationships within the community to promote the Care Finder program and assist with reaching potential clients.
- Supporting clients to understand and connect with required services, including My Aged Care, using warm referrals, and monitoring client engagement.

Qualifications
In addition to a self-directed passion and drive to support other within the community, you'll have:
- Experience working with older adults within the community with an understanding of the supports and services that are available in the local community and the process to access these supports.
- A commitment to delivering a person-centred approach.
- Understanding of the policy and funding environment of the aged care and community services sector along with a working knowledge of current legislation.
Mandatory Qualification/s, Competencies and/or Licences:
- Qualification in Social Work, Human Services, Aged Care, Community Services or Health and/or relevant experience
