Responsibilities will include:

• Provide case management to individuals living with a disability and/or mental illness in keeping with the guidelines contained in their service agreement and support plan.
• Utilise Evidence Based Practice in the development and implementation of clients individual support plans.
• Assisting in reporting, auditing and continuous improvement activities
• Liaise with external services agencies to ensure safe implementation of care plan goals
• One-on-one client facing work including medication assistance and basic personal care.
• Assist in new client sign-ups by providing appropriate information to prospective clients.
• Participate in project work as directed by the organisations senior management.
• Complete, maintain and store documentation in a confidential manner.

About you:

The successful applicant will have:

• Tertiary qualifications in a relevant field or be studying towards such a qualification; OR Relevant experience within the Community Services sector or experience in a relevant industry with transferable skills.
• Have a willingness to learn on the job.
• Effective and professional interpersonal and communication skills
• Effective problem-solving skills for complex clients
• Effective teamwork skills
• Accurate and timely decision-making skills
• Ability to work in a fast-paced environment.
• Solid understanding of Outlook, Excel, Word and Accessing Apps on your mobile phone
