Responsibilities will include:
- Provide case management to individuals living with a disability and/or mental illness in keeping with the guidelines contained in their service agreement and support plan.
- Utilise Evidence Based Practice in the development and implementation of clients individual support plans.
- Assisting in reporting, auditing and continuous improvement activities
- Liaise with external services agencies to ensure safe implementation of care plan goals
- One-on-one client facing work including medication assistance and basic personal care.
- Assist in new client sign-ups by providing appropriate information to prospective clients.
- Participate in project work as directed by the organisations senior management.
- Complete, maintain and store documentation in a confidential manner.

About you:
The successful applicant will have:
- Tertiary qualifications in a relevant field or be studying towards such a qualification; OR Relevant experience within the Community Services sector or experience in a relevant industry with transferable skills.
- Have a willingness to learn on the job.
- Effective and professional interpersonal and communication skills
- Effective problem-solving skills for complex clients
- Effective teamwork skills
- Accurate and timely decision-making skills
- Ability to work in a fast-paced environment.
- Solid understanding of Outlook, Excel, Word and Accessing Apps on your mobile phone
