Qualifications & experience
- Bachelor's degree in social work, Healthcare management or related field is desirable with 2 years experience in a similar position and proven experience in Client Services management, preferably in the health care or disability sector.
Tasks & responsibilities
- Client Relationship Management: building and maintaining strong relationships with clients, their families and caregivers as the main point of contact for client inquires and concerns.
- Program Coordination: collaborate with internal teams to ensure the seamless delivery of services and develop strategies to enhance effectiveness and client outcomes.
- Assessment and Case Management: conduct initial assessment to identify specific needs and work with healthcare professionals to develop care plans and fortnightly reporting to stakeholders.
