We specialise in the aged care field, focusing on supporting vulnerable seniors who require comprehensive assistance in accessing care, allied health services, housing, and other essential resources to enable them to age safely at home with dignity and respect. Many of our clients lack support from caregivers or family members, face language or literacy barriers, struggle to comprehend information, and encounter challenges in decision-making.

About You
We seek an exceptional individual to establish strong relationships with clients, helping them implement personalised care and services. Your role will involve diverse tasks, including aiding homeless or at-risk seniors, arranging home care services, and exploring housing options for retirement or residential aged care. Each client is unique, and we take pride in delivering outstanding customer service while tailoring care and services to meet their specific needs.

Make a Lasting ImpactThis role entails close collaboration with the Managing Director and Operations Manager.
Key Responsibilities:
- Supporting clients in achieving wellness and autonomy.
- Assisting clients in their own homes to maintain independence and preferred lifestyle through high-quality care.
- Addressing social isolation issues and helping clients enhance their living conditions.
- Conducting initial assessments for new clients.
- Creating care plans with chosen home care providers.
- Coordinating safe, timely, effective, and client-centered care and services.
- Managing client caseload, monitoring progress, and closing cases upon service implementation.
- Coordinating care aspects with various agencies and providers.
- Advocating for clients' to help them make informed decisions on housing, care, and social options.
- Promoting quality, cost-effective interventions and outcomes.
- Building effective relationships and collaborating with stakeholders in the case management process.
- Liaising with centrelink, health, and government agencies.
- Maintaining client records in the CRM, filling out necessary forms accurately, and generating weekly client status reports.

Candidate Profile:
- Previous aged care, community services or health industry experience (min. 2 years).
- Qualifications in social work, health, or community services.
- Exceptional communication skills and a talent for relationship building.
- Dedication to hands-on service and genuine client advocacy.
- Autonomy, forward-thinking, and a proactive "can-do" attitude.
- Willingness to travel across Sydney.
- Valid Australian driver’s licence.
