About the roleWe are seeking a Housing Support Worker on full-time basis, within the Street To Home Program, is to provide assistance and support to individuals and families who are experiencing primary homelessness (rough sleeping) to achieve long-term, secure and stable accommodation.This is an outreach program, meeting with service users in the community. The role of the Housing Support Worker is to provide support, information, referrals and service linkages to assist the individual to address the underlying issues that have contributed to homelessness. By assisting service users to address contributing issues and obtain stable accommodation, the program aims to break the cycle of homelessness.Key responsibilitiesYou will be successful in this role if you can:Assess service user eligibility from referrals received (internal and external referral sources) Complete sign up to Street To Home (STH) Program, including required Salvation Army paperwork and STH specific documentsEnsure that service user is listed on the priority public housing wait-list under the National Partnership Agreement on Homelessness (NPAH)Support the service user to set goals and establish a case plan, utilising evidence-based toolsWork within a holistic case management model, incorporating; assessment, planning, facilitation, advocacy and reviewAssist service users to gain independent living skills, to assist in maintaining future tenancies and living independently in the communityAttend multi-agency case planning meetings, where appropriate This is a permanent full-time position, located in Northbridge, WA.
