Position Title: NDIS Intake Officer
Location: Hervey Bay (and surrounds as needed)

Position Summary:
As an NDIS Intake Officerat Tropical Transformations Pty Ltd, you play a pivotal role in ensuring the seamless coordination of NDIS plans and services for individuals with disabilities. You will be responsible for managing client interactions, plan implementation, and team collaboration to deliver exceptional, person-centered support. Reporting directly to the NDIS Service Facilitator, you will contribute to empowering individuals with disabilities to achieve their goals.

Key Responsibilities:
*Develop support plans consistent with NDIS plans to achieve participant goals
*Liaise with Finance Department to develop appropriate quotes
*Conduct initial support assessments and risk assessments
*Communicate effectively with stakeholders and provide monthly service reports
*Coordinate new participant services with NDIS Service Facilitators
*Update internal systems to track participant plans and funding
*Ensure high-quality communication and administration
*Champion customer experience and quality
*Follow operational plans and compliance standards
*Create a safe workplace and comply with risk management protocols
*Participate in quality improvement activities and ongoing professional development
Duties:
*Understand NDIS framework and principles
*Provide support consistent with Human Rights Act and Disability Services Act
*Work with challenging behaviours and restrictive practices
*Supervise and develop teams
*Support people with different types of disabilities and communication needs
*Provide clear explanation and guidance to achieve NDIS and personal goals
Required Education/Licenses/Certificates:
*Certificate IV in Disability Studies, Social Work, Community Services, or related field (or equivalent experience)
*Proven experience in NDIS plan management or related role
*Leadership abilities with team development experience
*Excellent communication skills and understanding of NDIS framework
*Proficiency in Microsoft Office Suite and relevant documentation software - Nightingale an advantage
*Current First Aid and CPR certification
*Working with Children Check (Blue Card) & NDIS Worker Screening Clearance (Yellow Card)
*Valid Driver’s License, reliable car with registration and comprehensive insurance
*Valid Police Check
