EPIC Assist is a dynamic and caring not-for-profit organisation delivering the Disability Employment program throughout Queensland, New South Wales, and Tasmania. In addition, EPIC Assist also provide NDIS Support Coordination Services throughout Tasmania.

EPIC Assist is committed to creating a supportive and inclusive environment for everyone and we are seeking a dedicated NDIS Coordinator of Supports to join us on this mission to build our Queensland service.
About the role
As an experienced Support Coordinator, you will play a pivotal role in building our Queensland service. This role is Part Time 24 hours per week (Negotiable on days worked).
Your responsibilities will include:
- Using your previously established networks, collaborate with community groups, service providers and health care professionals to attracts clients to our service.
- Assessing the needs of individuals and developing personalised support plans.
- Coordinating with various stakeholders, including clients, families, and service providers.
- Monitoring and evaluating the effectiveness of support plans, adjusting as necessary.
- Advocating for clients to ensure they receive the best possible care and resources.
- Providing guidance and support to clients in navigating available community resources.
About you
You are a passionate, welcoming, and resilient professional, with a proven track record in Support Coordination. You will have strong experience building, supporting, and enabling positive relationships with participants and their support networks, and linking participants to mainstream or community services (i.e., housing, education, transport, health).
You will also possess:
- A recognised qualification in disability services or similar would be an asset
- Skilled in reading, understanding, and implementing NDIS plans.
- Excellent interpersonal and communication skills.
- Knowledgeable of the NDIS Practice Standards and the industry current best practices.
- The ability to complete comprehensive case notes in a timely manner.
