Our facilities and services are world class and our staff are dedicated, innovative and caring.About the roleWe are looking for an experienced Palliative Care Clinical Nurse Consultant (CNC). This position is a district role and the successful candidate will be required to travel and work across all facilities within Sydney Local Health District.The Palliative Care CNC provides expert clinical advice to patients, carers and other health care professionals for patients with chronic conditions who have end of life and Palliative Care needs. This includes, but not limited, to the following:
- Develops implements and evaluates care management plans for patients with chronic conditions with end of life and Palliative Care needs with complex health needs
- Provides leadership in the ongoing development of clinical practice
- Initiates and utilises findings of research in the provision of clinical services.
- Contributes to the development and delivery of specialty related education programs
- Participates in formal processes for the strategic and operational planning for the clinical service.
- Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field.
- Holds approved postgraduate qualifications relevant to end of life and Palliative Care or such other qualifications or experience deemed appropriate.
- Current NSW Driver's Licence (P1 and P2 acceptable).
