Reporting to the Commonwealth Psychosocial Support (CPS) Team Leader, this role is responsible for providing psychosocial support and assisting clients in accessing the NDIS. This is a non-clinical community-based supports that aim to facilitate recovery in the community for people experiencing mental illness – through a range of services to help people manage daily activities, rebuild and maintain connections, build social skills and participate in education and employment.
Other responsibilities include:
- Developing individualised recovery plans that facilitate choice and control by the individual
- Linking clients to other services, supports and community networks as required
- Supporting clients in navigating the NDIS application process where appropriate
This opportunity is 75 hours per fortnight, working Monday to Friday.
The successful candidate will be required to travel across the Perth metropolitan area and you will be required to have an NDIS Screen Check.
This opportunity is fixed term commencing on 22nd April 2024 date until 8th May 2025.
About You
To be considered for this role, you will possess;
- Minimum Diploma/Tertiary qualification in Social Work, Psychology or other human service discipline.
- Knowledge of NDIS practices and application process
- Demonstrated experience in mental health work and a sound understanding of recovery within a human services context.
- Knowledge and understanding of the spectrum of challenges that people face in accessing and maintaining links to health services
- Demonstrated experience in developing and maintaining positive relationships with a wide range of internal and external stakeholders including clinical services
- A valid First Aid
- A current Drivers Licence
- A current Police Clearance
- A current NDIS Screener Check
