About the role
GEM@HOME/REHAB@HOME is a bed substitution model and has received funding to expand the service. The team provides high quality flexible health services to patients in their own homes in the community.
We provide care and support to GEM and Rehab patients that have conditions that require short term directed care at home. This is a fantastic opportunity for an enthusiastic, passionate and dynamic Social Worker to join the expanding GEM@Home/REHAB@HOME program, to deliver early rehabilitation service in the home as part of a multidisciplinary team. As the successful applicant, you will closely with the Social Workers on the team to deliver great patient care and promote an integrated service that is consistent with the mission and values of Eastern Health.
About you
To be successful in this role you will possess:

• Eligibility for membership of the AASW
• Evidence of current CPE, as per AASW requirements
• Highly developed interpersonal skills
• Conflict resolution skills
• The ability to lead complex discharge planning
• Excellent time management skills
• A strong commitment to high quality care
• The ability to work as part of a diverse team and also work autonomously as required
• Current Victorian Drivers licence
