We are now seeking a proactive, results-driven Support Coordinator to join our innovative team in Melbourne.

The Role: 

In this dynamic and fast-paced role, you will be pivotal in navigating and managing the complexities of NDIS plans for our participants. Your days will be varied and demanding, requiring a proactive approach, quick decision-making, and a drive for high-quality results. 

Your Key responsibilities include:

  • Strategically Managing Client Needs: Oversee 25+ clients, tailoring plans that align with their goals, and ensuring quality service delivery within budgetary constraints.
  • Driving Innovation and Solutions: Utilize your analytical skills to develop creative, out-of-the-box solutions for enhancing service delivery and addressing funding challenges.
  • Leading with Excellence: Command the planning and execution of care strategies, setting high standards for yourself and others, ensuring the delivery of top-tier services.
  • Agile and Effective Communication: Rapidly respond to client and team communications, managing expectations and building strong, effective relationships.
  • Rigorous Quality Control: Meticulously monitor service standards and budget allocations, ensuring precise and thorough management of resources and outcomes.

You are a natural leader, keen on controlling the big picture while paying attention to the detail. Your style is direct, candid, and you are comfortable in a role where your day is filled with varied, fast-paced activities. You excel in environments where you can independently drive progress and are energized by opportunities to lead change and innovation.

A Day in the Life: 

As a Support Coordinator at True Connections, your day is a blend of proactive problem-solving and strategic planning. You’ll start your day reviewing emails and prioritizing tasks, ensuring timely responses within 48 hours. Each day brings new challenges, from managing budgets and addressing service quality issues to negotiating with providers and advocating for clients’ needs. You’ll conduct regular meetings with clients and their care teams, adapting to their needs and ensuring their plans are on track. Your expertise in NDIS processes and legislation will be crucial in guiding clients through plan changes, with flexibility to address urgent matters swiftly. Balancing admin days with fieldwork, you’ll connect directly with clients and stakeholders, ensuring a holistic approach to service coordination. Your role is independent yet collaborative, requiring creative thinking to research and propose new opportunities that add value to your clients’ lives.

Key Qualifications:

  • Demonstrated experience (2-5 years) in support coordination, preferably within the NDIS framework.
  • Tertiary qualifications in health, education, or community services.
  • Exceptional ability to manage multiple priorities, demonstrating both strategic thinking and hands-on execution.
  • Outstanding problem-solving skills, with a track record of implementing innovative solutions.
  • Strong communication and negotiation skills, with the ability to engage effectively across diverse groups.
  • Proven self-motivation and the ability to operate independently with high standards.
