About the opportunity
The Support Coordinator will be responsible for ensuring our customers are supported to achieve their goals as identified in their NDIS plans, enabling greater independence!
In this role you will:
Build capacity and implement all supports in a participant's NDIS plan
Report back to the NDIA on the outcome and goals, as required
Coordinate supports for individuals and families within their homes and in the community
Collaborate and liaise with internal and external service providers
Have a demonstrated understanding of the NDIS and experience managing funded plans
What we are looking for:
Previous experience building effective relationships with members of various local communities
Demonstrated experience in interpreting and implementing NDIS plans
Experience working with people with disability and their support networks
Demonstrated experience in providing support using enablement and strengths-based approaches
Tertiary qualifications in a relevant field
Driver's licence; travelling within the region will be required
NSW Working With Children Check or willingness to obtain
NDISWC
Current First aid and CPR
Experience using the Carelink system will be highly regarded.
