About the Role The community services team is a multi-disciplinary team that provides acute and post-acute care via hospital avoidance (MAPS) and hospital in the home (GEMRHITH) interventions. Care will be provided across seven days, in various settings including hospital clinics, community centres and consumers places of residence. The purpose of this position is to provide operational management, clinical practice supervision and strategic leadership for a multi-disciplinary community services team. The position delivers advanced acute, post-acute and hospital avoidance services to stakeholders/consumers within Metro South Health@Home and Queensland Health to achieve excellence in patient care, education and research. This position will report operationally to the CST Lead. This includes: Leading and managing specialised community-based services (may be services of a complex nature), including the provision of operational advice to multi-disciplinary teams. Utilising advanced level of clinical knowledge, problem solving skills and experience of contemporary acute and post-acute practice. Provide MSH-wide professional and operational advice on hospital in the home and hospital avoidance clinical areas and service delivery models. Working hours for this position are Monday – Friday, however the Community Services Team operates as a 7 day service, 7am-7pm, Monday to Friday and 7am to 4pm weekends.
