About the role
The Care Coordinator serves as a conduit between the patient, nominated carer, the treating clinical team and the Psychiatrist, providing clinical care and coordination for the patients allocated to their VMO(s) across all service settings. As an integral member of the hospital management team, responsibilities include assessment and screening, collection of collateral information from referrers and family, goal setting and discharge planning, education sessions with the patient and their nominated carer, discharge planning and outcome studies.
Essential criteria:
- Registration as a Registered Nurse with AHPRA and a current annual practicing certificate
- Minimum of five years clinical experience in a mental health setting
- Problem solving ability and triage skills
- The ability to coordinate and manage a number of concurrent projects
- Effective time management
- Excellent communication and liaison skills
- Excellent documentation and report writing skills
- Ability to remain calm and professional, and to work collaboratively with consumers, carers, staff, hospital accredited professionals and members of the public
Desirable criteria:
- Experience working in a private hospital setting
- Applicants with post graduate qualifications in mental health will be highly regarded.
