In this role, you'll play a crucial part in enriching the lives of individuals in the community. Operating on the IHCS Model for improved outcomes, you'll be responsible for coordinating care services and ensuring each person receives personalised support, promoting independence, inclusion, and social-economic participation. With a significant emphasis on clinical governance, you'll ensure care standards are not only met but exceeded. This position is based in Northlakes and offers Monday to Friday office hours with some travel required.
Responsibilities:
- Provide clinical leadership and drive business opportunities within social and community health settings.
- Develop and update comprehensive support plans with clients.
- Offer clinical supervision, assessment, and recommendations.
- Advocate for medical needs within the Support and Services team.
- Ensure organisational compliance with accreditation and standards.
- Lead clinical governance and manage quality frameworks.
- Implement regular meeting schedules and performance reviews.
- Monitor staff work practices and provide guidance.
- Participate in quality improvement and risk management.
- Provide direct and indirect clinical care to recipients.
- Apply advanced holistic care skills within a Case Management framework.
- Contribute to program development and planning.
- Report on individual and organisational KPIs.
- Supervise and coordinate support and healthcare workers.
- Maintain oversight over clients' outcomes and changes.
- Bachelor of Nursing degree qualification and AHPRA registration (essential)
- Advanced clinical nursing expertise.
- Demonstrated work experience with service delivery team on Restrictive Practice and human rights framework.
- Current or recent wound management experience, IDC/SPC insertion, and pressure injury prevention and management experience preferred.
- Knowledge and experience of community-based care and support services.
- Certificate in Geriatric and/or Psychiatric Nursing preferred.
- Highly developed assessment skills and creative problem-solving abilities.
- Innovative and dynamic approach to care provision.
- Strong networking, liaison, and relationship-building skills.
- Ability to maintain strong links with relevant service providers and users.
- Familiarity with Industry Quality Checks and standards.
