About this opportunity
Are you looking for a care management role where you can make a real difference in the lives of the ageing community through a relationship-based approach to care that supports older adults by enabling them to remain living independently at home?
As a Care Manager, you will be responsible for successfully directing and coordinating all activities associated with the care of potential and existing clients. This role provides leadership and management of the overall client experience and coordinates effectively with the office team. A Care Manager will have the best interests of the client at heart while supporting the achievement of operational goals.
The opportunity to provide:
- trust-based relationships with prospective and existing clients and their families.
- regular client visits (in person and/or over the phone) to ensure the highest level of care is being attained.
- knowledge and assistance to clients about a wide range of tailored resources and local services
Care Managers will be responsible for service enquiries, care consultations, client onboarding and reviews and care management of clients.

Key Responsibilities:
- Planning, managing, coordinating, and reviewing client goals.
- Actively communicate with team members and clients to develop care plans.
- Formalise an agreement with the client and the client’s family about the Care Services to be provided and the costs involved.
- Accurately document all interactions held with the client, family or other stakeholders about the client and share relevant information with appropriate team members including feedback about CAREGiver Performance.
- Meeting with clients and their families face to face to discuss and review current care plans and goals.
About you:
- Have relevant qualifications and experience in care planning and managing Care Services within the aged, community health or social services. (Candidates with a nursing background are preferred).
- Be an effective communicator with excellent customer service skills, demonstrated problem-solving skills and proven time management skills.
- Have a strong understanding and demonstrated knowledge of the Australian Aged Care Quality Standards and Home Care Package Management.
- Be highly efficient and organised with proven time management skills.
- Have a valid driver’s license.
- Relevant IT and administrative skills.
- Previous and relevant experience working with older people or in aged care.
- Able to work under pressure and multitask shifting priorities.
- Strong written and verbal communication skills
- Must hold a valid Australian Driver’s License
